Apple have made the process of upgrading your iPhone pretty easy thanks to iCloud Backup. This page is to document the apps I have had issues with when upgrading.

Troublesome

Google Authenticator / 2FA apps

Google Authenticator, Authy and other 2FA apps - be sure to make sure that every 2FA account you use has a fallback SMS number setup otherwise when you get rid of your old phone, you might not be able to access accounts on your new phone.
